A Cromwell man (47) has been charged with three counts of dangerous driving causing death and injury, after the truck he was driving collided with two vehicles on State Highway 6 between Kingston and Frankton.
An Invercargill church pastor was killed and her daughter and a pregnant woman were injured in the accident, on February 25.
Joanne Steel (40), pastor of the Southern River Church in Invercargill, died instantly when the concrete truck veered across the centre line into Mrs Steel's Nissan Terrano 4WD and another car about 6.20pm, near Wye Creek, Detective Lisa Watt, of Queenstown, said.
Mrs Steel's 14-year-old daughter, who was travelling with her mother, was injured, but not seriously.
Det Watt said the 21-tonne concrete truck was travelling towards Queenstown when it crossed the centre line and hit the oncoming vehicles.
The truck hit Mrs Steel's 4WD and the car behind.
Both vehicles ended up about 50m off the road in bushes and Mrs Steel was killed instantly.
The driver of the second car, a pregnant Garston woman, was taken to Dunedin Hospital for treatment.
Both survivors suffered moderate injuries.
The teenager spent two weeks in hospital and the pregnant woman was released after five days.
A Serious Crash Unit investigation has concluded and Det Watt said the man, who was uninjured, would appear in the Queenstown District Court next Monday.
