Wanaka-based adaptive skier Adam Hall won the senior sportsman title.
Samuels (30) was named senior sportswoman of the year before she collected the Bruce Grant Memorial Trophy.
The 2004 Otago University graduate had an outstanding 2012 season, winning the New Zealand elite road cycling title before heading into her multisport season.
In International Triathlon Union World Series races she took four top 15 placings, including a gold medal at the World Cup in South Korea. She was 35th at the Olympics.
Hall, also from Wanaka, attributed his successful 2012 season to a gruelling training programme, constant travel overseas, time in the gym and trial of new equipment.
His dominant season in his main slalom discipline took place in the Noram Series and World Cup events. He finished the year with with 12 gold medals. Team of the year award went to The New Zealand men's curling team, three of which come from Central Otago. Sean Becker, Scott Becker and Phil Dowling were part of the five-strong team that played all of the 2012 season together and finished the season with a world ranking of 12th.
The junior sportsman award went to Alexandra track cyclist Liam Aitcheson, who won the national championship title in the under 19 points race and team pursuit and fourth in the individual pursuit.
He was selected for the New Zealand team to compete in the Junior World Track Cycling Championships, where his team was second in the team pursuit and Aitcheson finished fifth in the points race.
Another top young cyclist, Sophie Williamson from Alexandra, was named junior sportswoman of the year. Wanaka multisport coach Val Burke was named coach of the year and the John Fitzharris memorial services to sport award was won by Queenstown ice hockey stalwart Charlie Reid.
