Practice manager Ali Wilden showed the Minister around and explained how online patient portals are an early success of the programme – helping patients access their records, check results and be more engaged in managing their own health.
“We’re committed to improving our services for our patients and with an increasing number of patients with more complex health needs, we need to ensure we can operate our practice to meet their needs as well as maintain a viable practice into the future,” says Ali.
Gore Medical is one of 16 general practices to have been selected by Southern DHB and WellSouth Primary Health Network to become Southern Health Care Home practices.
The Health Care Home model promotes access to urgent and unplanned care, provides more preventative care and better support for people with complex needs.
THE HCH initiative is part of SDHB’s Primary and Community Care Strategy and Action Plan, a blue print for improving the integration and the delivery of health services across the district.
