The title was Caversham's 10th in the last 11 years, after a drawn-out season in which it only dropped two points when it was held to a 2-2 draw with Dunedin Technical on May 1.
Caversham coach Steve Fleming was ecstatic with the season, which also included a place in the Chatham Cup semifinals.
"The team went from strength to strength," Fleming said.
"Seventeen league wins and one draw, and a cup run that had clubs from throughout the country not wanting to play us.
"With a little luck on our side, the Chatham Cup final was certainly within our grasp.
"It's my 14th southern league title as a player or coach, so I'm very happy."
Grants Braes defended well in numbers on Saturday and looked to maintain possession and build from the back when Caversham's attack broke down.
In the 27th minute, an attacking turn by Robbie Deeley inside the area left Braes defender Liam Lockhart flat-footed, and a careless trip on Deeley had referee Roberts pointing to the spot.
Deeley struck the penalty firmly, but Braes keeper Josh Columbus saved well to keep the score at 0-0 at the break.
The second half had Caversham continuing to apply pressure on the Braes goal and the match opened up in the 53rd minute.
Robbie Deeley evaded tackles and distributed an accurate pass to the unmarked Harley Rodeka, who cut inside and slotted a 15m shot into the top left-hand corner.
Ten minutes later an innocent through ball from Braes' left flank turned into an attacking threat when Caversham defender Tim Horner lost his footing and allowed Braes striker Ash Hodges through on goal, to finish well and level the scores.
The match was settled in the 70th minute after a poor goal kick from Braes defender Ben Elder went straight to Caversham midfielder Blair Duncan.
Moments later Braes was picking the ball out of the net after a precise pass by Duncan found striker Ant Hancock, who had made an incisive run to flick the ball past the advancing Columbus.
Robbie Deeley claimed Caversham's third in the 85th minute after a good cut back from Harley Rodeka wrapped up a quality performance and win for Caversham against a tough opponent.
