Police and a St John am-bulance were called to Ar-thurs Point Rd, Queenstown, about 9.30am yesterday.
The woman (24) was treated on the verge for about 15 minutes before being loaded on to a spinal board and taken to Lakes District Hospital by ambulance.
She was conscious and able to speak to St John staff while being loaded on to the ambulance.
Kelvin Perriman, territory manager for St John Central Otago-Southland Lakes, said: ''She was assessed by medical staff and then helicoptered to Dunedin Hospital for treatment.
''It was a serious head injury.''
Sergeant Blair Duffy, of Queenstown, confirmed the vehicle was moving when the woman exited it, near the junction with Atley Rd about 1km from the Edith Cavell bridge over the Shotover River.
''It's a domestic incident,'' Sgt Duffy said.
''Inquiries are continuing.''
