Dr Farry, who was awarded an MNZM earlier this year for services to rural medicine, died on October 9, at Twizel, while working as a locum.
He was 65.
Dr Farry began practising as a GP in Queenstown in 1971 and soon after began teaching students and registrars here.
He dedicated nearly 40 years to rural medicine and education for general practitioners.
Dr Richard Macharg, of QMC, said the skilled doctor, educator and organiser at a national level "was first and foremost a man of great compassion, who put patients first and enjoyed the time he spent helping people above all".
QMC Charge Practice Nurse Liz Taylor said the centre decided to establish the book of remembrance to give the community the opportunity to share their memories.
"It may well go to the family," she said.
"Pat was the founder of the medical centre and really established QMC as it is.
"He was so full of doing things. He made a huge contribution, but he hadn't finished.
"We knew a lot of patients would want to pay their respects and make note of that, where they might not necessarily have gone to the funeral."
The remembrance book is located at the main QMC building in Isle St, Queenstown, and will stay there for about a month.
