Fatal fall among SDHB 'adverse events'

An unwitnessed fall resulting in death was one of the 35 serious adverse events reported at the Southern District Health Board in the past year.

The serious adverse events report released at noon today covered the period from July 1 last year to June 30 this year.

Clinical processes accounted for 34% of the 35 events, falls 29%, clinical administration 23%, medication errors 8%, behaviour 3% and medical equipment 3%.

The falls resulted in a death, shoulder dislocation, fractured left hip, fractured necks, fractured pelvis, fractured acetabulum, a facial fracture, head injury and skin tears.

A formal review was initiated after the unwitnessed fall and subsequent death.

Adverse events during clinical processes included an unexpected death during a procedure and an unexpected inpatient death.
