Family members of the victims arrived in Christchurch on Sunday and were presented with the difficult situation of whether to turn off life support for one of the victims who was critically injured.
The Malaysian family of five had been on holiday when a truck collided with their car on State Highway 1 around 3.15pm on Friday, killing the mother, a family medicine specialist, her husband and their middle daughter.
Their youngest daughter, believed to be about 12, was taken to Wellington Hospital and the eldest, 15, was taken to Christchurch Hospital where she was put on life support.
The Herald understands the fourth person who died was the teenager in Christchurch.
Police said the fifth person remains in hospital in a serious but stable condition.
Sam Yau, president of Canterbury Malaysian Association, who visited the 15-year-old on Saturday said five members of the family arrived from Malaysia on Sunday "with difficult decisions to make".
They met with doctors to decide on whether to turn off life support and also met with other authorities to discuss burial plans.
The family, from Sabah in East Malaysia, had been in New Zealand since December 2 and was believed to be driving towards Christchurch to visit the mosque where 51 people were shot dead in March when the accident happened.
The accident led to the closure of both sides of the main South Island highway.
Two Kaikoura fire crews used cutting equipment to free the surviving passengers and remove the bodies from the car.
Three of them died at the scene, while the two girls were flown via rescue helicopter to Wellington and Christchurch hospitals.

About 5.50am on Friday there was a crash between a motorcycle and a logging truck on State Highway 5 at Te Pohue.
One person died at the scene but a second person succumbed to their injuries on Saturday evening.
Hastings police want to hear from anyone who was driving in the area between 5.30am and 6am on Saturday, especially anyone who saw the motorcycle.

Two people died in a car after it was struck by a train at the intersection of Piako Rd and State Highway 26 at 4.10am Saturday.
SOUTH AUCKLAND:
A pedestrian was fatally struck by a train in South Auckland on Saturday. The crash happened on Hospital Rd, Mangere about 12.30am.
WHAKATANE:
A person died on Wainui Rd near Whakatane at 5.15am Sunday after their vehicle rolled.
REPOROA:
A motorcyclist died on Broadlands Rd, Reporoa, near Rotorua, after a crash about 1.30pm on Sunday.
Police said traffic was being diverted and motorists are advised to avoid the area if possible.
OHAUPO:
A child was killed after being hit by a car just before 3pm on Sunday in a driveway on Hams Rd.
